The Primary Care Wellbeing Service commissioned by the Bradford District and Craven Clinical Commissioning Groups has won a national Positive Practice in Mental Health award for primary and secondary mental health care collaboration.
The service, provided by Bradford District Care Trust, was recognised for its partnership working and innovative approach. It runs from four GP practices across the Bradford district: Kilmeny Group Medical Practice in Keighley; Tong Medical Practice; Moorside Surgery and Clarendon Medical Centre in Bradford.
The team, working in partnership with GPs and the practice team, includes a consultant clinical psychologist, occupational therapist, physiotherapist, EMDR therapist, specialist dietician and a consultant psychiatrist. Staff work with patients with long-term illnesses or chronic diseases, and who frequently visit their doctor with physical and mental health problems, aiming to:
- better support the patient’s long-term conditions or illness
- improve patient care and clinical outcomes
- reduce unnecessary hospital admissions
- improve the quality of care
