West Yorkshire and Harrogate Health and Care Partnership Frailty Workshop

The West Yorkshire and Harrogate Health and Care Partnership is developing a clinical strategy to provide an overall framework for its work.  One element of the work to develop the clinical strategy is to understand the services provided to support people across our area.  We are looking at frailty for this particular workshop.

The Partnership would like to invite you to a workshop to explore the "Frailty Journey" for people in our area. We want to assemble a representative regional group of practitioners to influence and improve the work we are undertaking.

During the last three months, an information gathering exercise has been undertaken across all six places in the West Yorshire and Harrogate Health and Care Partnership area.  Place is the primary unit of focus for responsibility. Where the Partnership adds value is in comparing and contrasting, sharing best practice and bringing places together to share learning.

The exercise has focussed on frailty; we have explored the patient journey using five virtual people to enable us to gain a deeper insight and awareness of services and specialties that are available in each place.  Throughout the process, the only variable has been place.  We have taken each of our virtual people on a holistic journey through each of the six Places, to explore their experience.  Consideration has been given to:

  • best practice
  • services accessed
  • services that were available but may have not been accessed
  • any potential missed opportunities. 

Aspirations of care providers and people who access services have also been reviewed.  The information from the meetings held has been collated and an overview will be presented at the workshop.

You may have already been part of this work through interviews with a wide range of clinicians and other stakeholders, including the voluntary sector and Health Watch, which will be ongoing until early February 2019. To further enrich the information gathering exercise we have also spoken directly with service users and people across WYH.

This workshop is a key opportunity for us to feedback on the valuable work which you have contributed to.  An agenda will be provided on the day, however it will focus on the evaluation of the Frailty Journey and review good practice and discuss any gaps highlighted.
We would be delighted if you could attend this workshop to not only review the findings, but to work together in groups to review good practice and discuss the evaluation which will be used to inform the WYH HCP clinical strategy and the response to the new NHS Long Term Plan.
