NHS Healthier You: Diabetes Prevention Programme

Are you at risk of developing type 2 diabetes? The NHS Healthier You: Diabetes Prevention Programme, one of the national programmes available in West Yorkshire, is here to support you. This nine-month, evidence-based lifestyle change programme is designed to help you take control of your health and reduce your risk. Choose the option that works best for you: join a face-to-face group or access the programme digitally—it's your choice. 

Description
  • Free sessions and tips to help reduce your chance of developing type 2 diabetes
  • Delivered face-to-face at local venues, via digital app and remotely for some participants
Criteria
  • Aged 18 or over 
  • Blood test in the last 12 months which shows that your blood sugar is higher than normal
  • History of gestational diabetes mellitus (GDM)
  • Benefits need to outweigh risk if you’re aged 80 or over
Exclusions
  • Pregnancy
  • Current or previous diagnosis of type 2 diabetes
Referal process
  • Speak to your GP or healthcare professional

John's video diaries

We asked John Ebo to keep a video diary of his journey through the NHS Diabetes Prevention Programme. 

  • Part 1 - John describes how he felt when he found out he was ‘pre-diabetic’ and what he plans to do about it

  • Part 2 - John describes what resources he accessed to find out more about reducing the risk of diabetes and his initial thoughts about embarking on the NHS Diabetes Prevention Programme

  • Part 3 - John has completed his first video session and tells us what he thinks so far. He talks about the other people in his group and the coach’s advice around diet and weight

  • Part 4 - John is 5 months into his 9-month journey and loving it. He explains how nothing has dropped his blood pressure like this programme. With 5kg lost, John is taking it steady and well on target to lose 10% of his original body weight. John has lots of great tips for others wanting to reduce their risk of type 2 diabetes and boost health and wellbeing

  • Part 5 - we find John eight sessions down and five to go. It’s December and perhaps the trickiest part of the journey with all the temptations that the festive period brings. John recaps on what he’s learned so far and describes how the most recent session looked at recognising cravings and managing the hunger scale

  • Part 6 - John is seven months into the nine month programme and reflects on the last session which covered cooking, eating out, shopping and embedding good habits into your everyday life to prevent diabetes – and it doesn’t need to cost the earth if you use your local market and stick to seasonal fruit and veg

  • Part 7 -  the last instalment of John’s video diary detailing his journey on the Healthier You NHS Diabetes Prevention Programme. What a journey – and what a difference. John describes how it’s gone and share his latest stats. The results speaks for themselves
