VCSE Power showcase - Day 2: Co-designing social prescribing pathways and personalised care

Social Prescribing is a term used to describe a range of non – medical options that can improve our health. These activities are usually happening in our local communities and can include art groups, walking groups, sport, libraries, cooking, faith groups etc. The aim is to build on whatever a person is interested in to help improve their physical and emotional wellbeing. Therefore, it links with personalised care which emphasises that we are all different and need to be involved in deciding what support we receive.

The VCSE has worked in partnership with agencies and residents to develop pathways that help individuals know about and access local support.

Space2 are working with Feel Good Factor and Yorkshire Cricket Foundation alongside community collaborators across the city, on a ‘social prescribing programme’ to deliver activities, which tackle social isolation, improve wellbeing - and specifically support the communities of Chapeltown, Gipton and Harehills.

The Patient Ambassador Programme

Nathan is a Patient Ambassador for South Leeds communities and has a really interesting role as a social prescriber. He is striving to do better every single day for the good of his patients.

Social prescribing helps people to address concerns in their life that are having a negative effect on their physical and mental wellbeing.  It could be debt problems, housing issues, loneliness…. or anything else.  It’s tailored to the individual and connects them to services that help them feel better and live healthier lives. Social isolation, emotional issues or wellbeing concerns can cause all sorts of conditions including anxiety, depression, stress and many more conditions. By supporting people it can stop them even needing to visit a GP or go to A&E. This is great for the person and for the NHS.

 

Linking Leeds - Missing Link Podcast

The idea for the Linking Leeds podcast came from members of our People’s Voice Group, and they designed the questions for our host. Many people need help to improve their wellbeing, but often hit barriers to asking for it. In this podcast, we want to normalise and remove those barriers, so that everyone can talk openly and feel better. We will be talking to a variety of people to get their view on what barriers they face. Over the coming weeks the host will speak to people from all walks of life – such as Wellbeing Coordinators, GP’s, clients, other professionals and many more.
