The NHS Digital Weight Management Programme, is a free online app based 12-week programme. Digital support to join the programme may be available for those patients who don't have easy access to devices like computers, laptops or smartphones. Please see the digital access pages on the Healthier Together website to find the nearest services that offer digital devices, or refer to your local social prescriber service who can find local support.
Criteria
Aged 18 or over with hypertension or diabetes and BMI over 30 (27.5 for Black and Asian Groups who are more at risk)
Exclusions
- Severe or moderate frailty
- Pregnancy
- Eating disorder, unmanaged co-morbidity (more than one disorder or disease at the same time)
- Bariatric surgery
Referral process
GP practice or healthcare professional
So far the programme has received over 110,000 referrals from general practices to date. To continue making referrals and save time by ensuring more of the patient’s information auto-populates, general practices must now use version 2 of the referral form.
CCG IT teams are uploading this version onto your practice system. If your practice uses Ardens or DXS, version 2 is already available. You can also download the updated form.
