About the Long Term Conditions and Personalised Care Programme
People with long-term conditions have told the Department of Health that they want services that help them remain as independent and healthy as possible. By having access to information that is easy to understand, they feel they have a the knowledge, skills and confidence to understand and manage their conditions more effectively and support wider choices. Connecting people to communities is a key element of personaliesd care to ensure that people are able to self-manage their physical health conditions, whilst also actively improving their mental health and wellbeing. People indicated that they want more services delivered safely and effectively in the community or at home, with more proactive and integrated services personalised to them and their needs (Opinion Leader Research, 2006; DH, 2005a).

Our long term conditions and personalised care programme work with partners accross West Yorkshire, including the West Yorkshire Association of Acute Trusts (WYAAT), primary and community care and organisations from the voluntary, community and social enterprise (VCSE) sector. As part of the NHS England Personalised Care Demonstrator Programme West Yorkshire and Harrogate have been working with support from NHS England to build, develop and spread the model of personalised care delivered locally in Bradford District and Craven, Calderdale, Harrogate, Kirklees, Leeds and Wakefield.
