f you're a health and care professional, carer, or someone with an interest in improving health and care using digital technology, we'd love it if you could attend our event on the 28th of July. The workshop, held in collaboration with The Good Things Foundation, will look at how technology can be used to improve care for people with hearing or visual impairments.

When is it? Friday 28th July at 1-4pm

Where is it? Rooms 3 and 4, Laura Mitchell Health and Wellbeing Centre, Great Albion Street, Halifax HX1 1YW

The workshop is being held as part of the national NHS Widening Digital Participation programme, funded by NHS Digital. The programme aims to reduce digital exclusion in the UK, and ensure people have the skills they need to access relevant health information and health services.

West Yorkshire and Harrogate is one of 20 pathfinder sites nationally, and will be specifically focusing on the needs of people with hearing or visual impairments. The project will benefit from leading experts in the field, including Molly Watt, a patient leader, usability and accessibility consultant and keynote speaker, and mHabitat, experts in digital innovations in health and social care.

The inclusive co-design workshop is open to all and will be particularly relevant to those with an interest in:

·    Self-care
·    Accessibility
·    Care and services for people with a hearing or visual impairment
·    Digital innovations, including the use of wearables and mobile devices
·    Digital inclusion.

Saturday 10 March 2018, 11:00 am to 2:00 pm

This will be the second event with our stakeholders help inform the next stages of this work with meaningful conversations to co-produce a robust governance structure for West Yorkshire and Harrogate Health and Care Partnership.

Tuesday 10 July 2018, 11:00 am to 2:00 pm

Event

Tuesday 6 November 2018, 11:00 am to 1:00 pm

Event

Tuesday 8 January 2019, 11:00 am to 1:00 pm

Purpose of the event: to engage a wider and diverse group of health, public health, social care, PWLE,and VCSE partners in conversation about personalised care, what it is why it’s important, celebrating what we are doing in WYH, how to get involved and what the next steps are.

Starting the culture change to a different way of working.Core message of the day – if we want self-care PC to really fly we need to change the culture, change the way we engage with people, the conversations we have with each other, the relationship between the clinician and the patient, the way services are commissioned.Big picture – culture change, what can I do.how do we change the culture

Aims:

  • We (the primary care community and others leading transformation change in the system) understand the context of the WY&H HCP and the breadth of both challenge and opportunity for change
  1. We understand the breadth of expertise across the system and identify any gaps in knowledge or expertise
  2. Agree some practical next steps on the journey to establishing and delivering the personalised care agenda for the partnership – what should we ‘invest in’ and what would the ‘return’ be?
Monday 25 February 2019
