How will we know we have delivered?

Joint forward plan chapter cover - how we will know if we have delivered our planAmbition 1: We will increase the years of life and years lived in good health, with the greatest increases for those who currently experience the poorest outcomes.
  • Reduce smoking prevalence in West Yorkshire to 5% by 2028 with a focus on those in routine and manual professions.
  • By 2028 we will have improved the equity of access to treatment and support for people who are living with obesity.
  • By 2028 we will match, or better the national median percentage scores of the Sentinel Stroke National Audit Programme and we will improve compliance in relation to the 21 elements specified within the integrated community stroke service model.
  • We will reduce the gap in COVID, Influenza and Pneumonia vaccination uptake between communities ranked in the most and least deprived quintiles in West Yorkshire.
  • We will improve our diagnosis of hypertension and diabetes to reduce the gap in the treatment between communities ranked in the most and least deprived quintiles in West Yorkshire.
  • We will reduce the gap in life expectancy between people living in communities ranked in the most and least deprived quintiles in West Yorkshire by 10% by 2028.
  • Support our places to annually increase those taking up the national type 2 diabetes prevention and management programmes
  • Support our places to achieve the national average percentage for adults living with Type 1 or Type 2 Diabetes to meet the three treatment targets and the eight care processes for diabetes in 2023/24; and aim for an annual 1-2% increase for the next four years.
  • Increase uptake of continuous glucose monitoring, aiming for 80% uptake for type 1 diabetics and 70% for type 2 diabetics by March 2028.
  • Treat 77% of patients with hypertension to NICE guidance by March 2024 and realise continuous increase thereafter
  • 82% of patients with atrial fibrillation known to be at high risk of stroke to be adequately anticoagulated by 2024 with a 2% increase per place each year thereafter until 2028.
Ambition 2: We will increase our early diagnosis rates for cancer
  • By March 2028 Targeted Lung Health Checks will have been made of all appropriate citizens (currently 55%)
  • By March 2028 62% of cancer patients will be diagnosed at stages 1 and 2 (currently 54%), as progress towards the NHS Long Term Plan’s aim for these patients.
  • By March 2028 80% of patients referred with suspected cancer symptoms will either be confirmed to have cancer, or excluded from the cancer pathway within 28 day (currently 75%)
  • We will reduce the gap in stage 1 or 2 lung cancer diagnosis between people living in communities ranked in the most and least deprived quintiles in West Yorkshire.
Ambition 3: We will reduce suicide rates
  • We aim to achieve a minimum 10% reduction in the suicide rate across West Yorkshire over the next five years.
  • We will enable organisations within the WY HCP to reduce suicide among their staff and volunteers.
  • We will improve system-wide information sharing around suicide prevention, identifying, and sharing what works.
  • We will target resources where suicide risk is the highest for men and for people who are unemployed or in low-wage, skilled-manual and self-employed work.
Ambition 4: We will reduce antimicrobial resistance
  • We will continue to optimise patient outcomes and reduce the risk of adverse events by supporting healthcare professionals to prescribe the most appropriate antibiotic (the right antibiotic, delivered in the correct way, within the correct time, for the correct duration – and only when required).
  • We will continue to take collective action to minimise rates of bloodstream infections, and other healthcare associated infections, across West Yorkshire
  • We will deliver annual activity to raise awareness and understanding of infection prevention, educate about antimicrobial (antibiotic) resistance, promote appropriate antibiotic use, and encourages alternative treatments.
  • We will embed action to ensure that our citizens and healthcare professionals recognise the environmental impact of antimicrobials and take action to reduce carbon footprint.
Ambition 5: We will reduce stillbirths, neonatal deaths, brain injuries and maternal mortality.
  • Reduce stillbirth, maternal death, neonatal deaths, and neonatal brain injuries that happens when a baby’s brain doesn’t receive enough oxygen during delivery by 50% by 2025
  • Reduce the rates of pre-term birth to below 6% by March 24, and by a further 1% in each of the following two years
  • Ensure that every women will have an individual care plan that is co-created by March 24
  • By March 2024, all pregnant women will have access to smoking cessation treatment.
Ambition 6: We will reduce the gap in life expectancy between people with mental health conditions, learning disabilities and/or autism and the rest of the population
  • Reducing smoking rates for people with serious mental illness, learning disabilities and autistic people by increasing the numbers of individuals who successfully quit smoking in our inpatient and community settings
  • Co-producing and implementing a consistent service user experience measure, tailored to our services and settings
  • Tackling the disproportionate impact of race on access, experience, and outcomes in our services, aiming for 100% complete ethnicity recording across all settings and implementing PCREF guidance in full.
  • We will work towards offering regular Annual Health Checks to every person with a Learning Disability or Autism diagnosis in West Yorkshire.
  • We will reduce the gap in life expectancy attributable to four common physical health conditions (CVD, respiratory disease, liver disease and cancer) between the general population and those with SMI.
Ambition 7: We will address the health inequality gap for children living in households with the lowest incomes
  • We will deliver a multi-disciplinary Tier 3 ‘Complications from excess weight’ service to 100 patients in 2023/24, and work to extend the availability of this service into coming years with a focus on population groups in greatest need.
  • We will continue to roll-out vCreate, supporting children (and their families) to have greater self-management of their epilepsy through timelier and more effective liaison with their paediatrician - whilst at the same time reducing demand on clinic appointments.
  • By 2028 we will reduce the number of children in West Yorkshire presenting with dental cavities through reducing inequalities in dental access for children in West Yorkshire.
  • By 2028 we will contribute to a reduction in the number of children living in fuel poverty across West Yorkshire.
Ambition 8: We will have a more diverse leadership
  • We will work collaboratively to ensure that the proportion of staff across all levels of seniority, is reflective of the population and communities we serve. We will also work in partnership to support our diverse colleagues to fulfil their potential.
  • We will use our collective influence to ensure the diversity on decision making boards at place and at West Yorkshire level are reflective of the local population including VCSE advocates.
  • We will ensure the diversity of the leadership of the West Yorkshire Integrated Care Board is reflective of the local population and that we continue our work to improve the experiences of our staff.
  • We will ensure that we deliver the high improvement actions highlighted in the NHS equality, diversity and inclusion (EDI) improvement plan. 

Ambition 9: We aspire to be a leader in the delivery of environmentally sustainable health and social care through increased investment, mitigation, and culture change throughout our system.

  • We will deliver our partnership Climate Change Strategy and the Green Plan to be net carbon zero by 2038 and adapt our system to increase resilience and minimise vulnerability to climate harms.
  • By 2028 we will support the health and care system to achieve a carbon reduction of 50% from the 2019 baseline.
  • 80% of contracts and investments comply with sustainable procurement strategy and policy.
Ambition 10: We will support people in West Yorkshire to contribute to, and benefit from, economic growth.
  • By 2028 we will contribute to an increase in the percentage of employees in quality work across West Yorkshire.
  • By 2024 we will support the launch and delivery of the Fair work charter across West Yorkshire.
  • By 2028 we will move towards paying the real living wage for our staff including commissioned services.
  • We will deliver a career development project, ‘Project Hope,’ to 25 care experienced young people to support them gain employment in the health and care sector in 2023/24, and work to extend the availability of this service into coming years.
  • Every 18 months we will monitor and report on economic value of the VCSE across West Yorkshire ICB.
  • By 2028 we will increase the proportion of the West Yorkshire NHS budget invested in the VCSE.

Supporting metrics

In order to deliver statutory ICB functions and to enable the delivery of the partnership’s ambitions we have also identified the following supporting metrics:

  • Increase in the number of carers identified and coded in primary care
  • Increase the number of registered Working Carer Passports within organisations
  • Identify and increase the number of people across who had an electronic shareable Advance Care Plan in place before they died.
  • Identify and collate the percentage of people across West Yorkshire who had three or more emergency admissions in the last three months of their lives to inform and reduce health inequalities
  • Increase annually, the number of staff trained in personalised care
  • Drive an annual increase in the use of personalised care approaches
  • No people waiting for more than 65 weeks by the end of March 24
  • 25% reduction in out-patient follow-up appointments by the end of March 24
  • Improve A&E waiting times so that no less than 76% of patients are seen within four hours by March 2024 with further improvement in 2024/25
  • Improve category two ambulance response times to an average of 30 mins across 2023/24, with further improvement towards pre-pandemic levels in 2024/25
  • Monitor our collective adult general and acute bed occupancy to move us towards the operational planning target of 95.2% in 2023/24
  • All statutory organisations in West Yorkshire will be trauma Informed and responsive by 2028.
  • By 2028 we will contribute to the reduction of Accident and Emergency attendance in relation to serious violence.
  • Every 18 months between 2023 and 2028 we will monitor and report on the number of VCSE organisations in West Yorkshire.
  • Fuller Delivery Board will agree within 2023/24 the outcome measures to demonstrate the ICB’s success in effecting its response to the implementation of the recommendations set out in the Fuller report, aligned to specific work programmes as part of the delivery plan and implement these from April 2024 at the latest
  • Urgent community response services should consistently meet or exceed at least 70% of patients referred to them within two hours
  • Virtual wards will be utilised to at least 80% of their capacity by September 2023
  • Reduce the percentage of beds occupied by patients with no need to remain in hospital care
  • The ICB will agree within 2023/24 the outcome measures to demonstrate its success in delivering its dental priorities, namely: improving access, reducing inequalities in oral health, water fluoridation, and recruitment and retention of the dental workforce. These will be implemented by April 2024 at the latest.

It’s important to note that whilst all the measures outlined above will provide us with the information to help support monitoring of our plan, only those measures which are timely and publicly available will be published on our website at this time.
